Please read the contract and sign below to agree to our policies. This must be done before your child's first day of tutoring or program participation. If you'd like a copy for your records, then click on the PDF link below and print.
The Academic Achievement Center (AAC), does not provide child care in the place of parental care. Students are expected to participate in the activity they are signed up to attend.
The AAC will contact a parent, guardian, or another authorized person to pick up a student in the following situations: the student is not participating in the activity they are signed up for; the student is unhappy and chooses to discontinue participation; the student refuses to get on or off the STEM Shuttle; disruptive behavior; upon student request; illness or other emergencies.
Regular attendance is expected and necessary for programs to benefit any child. Students with excessive absences can lose their designated time if the AAC deems necessary.
We require 24-hours notice to cancel without charge. This amount of time allows for us to communicate with other clients and offer the time to another student. Our teachers are in high demand. Fewer than 24-hours notice will result in being charged for the session, even if we reschedule to a later time in the same week. This policy reflects the respect of everyone's time.
Cancellations should be made to the AAC by phone at 541-654-4999, or email at firstname.lastname@example.org. If a message is received in the office, 24 hours prior to the scheduled appointment time, there will be no charge. If the AAC is not notified 24 hours prior to the scheduled session time, then the client will be charged for the full amount of scheduled session.
Students will not be unfairly penalized for unsuccessful cancellation attempts that may result due to internal communication or staff errors.
Sick Child Cancellation Policy:
We understand that illness can occur suddenly. We encourage parents to cancel sessions when students are sick, and we firmly believe that when you are sick, your only job is to get well. Because of this understanding, we take on good faith when cancellations occur due to sickness. In order to qualify for this cancellation, the AAC needs to receive advanced notice, by 10:00 AM if the student is staying home from school, or 1 hour prior to the start of the session if they leave school during the day. If the AAC is not notified the session will be charged in full.
Does your student have any pertinent medical information? If yes, please specify in the box at the bottom of the page. Please include any allergies, medications, and special needs. This information will be kept confidential and only be accessed by staff on a need-to-know basis. If your child takes medications, please include how often medication should be administered as well as any other special instructions.
Behavior & Conduct:
Students are expected to work hard and stay focused while at the AAC. Students that are a distraction to others may be no longer allowed at the AAC.
Inclement weather policy:
The AAC follows 4J School District’s cancellation as far as school closures go. If 4J schools are closed then the AAC will cancel all tutoring sessions and after-school programs for that day. You will not be charged for sessions that are canceled due to weather.
3-to-1 tutoring, $38/hour.
1-to-1 tutoring, $53/hour.
Online tutoring, $53/hour.
Cohort tutoring, $53/hour
ACT/SAT Prep, $53/hour or prepaid package option.
Camps and classes, prices vary
For payment, please provide the AAC with a credit card during your first visit, or create an account on Oases and enter your credit card (you will receive an invite via e-mail to join Oases). Client credit cards will automatically be charged after each session. If clients prefer not to have your card securely on file, then they need to pay for each session on the day of that session or prepay at the beginning of the month to establish session credit that will be used for that month.
Good Fit Tutors:
Please contact the AAC if you do not feel like your assigned tutor is a good fit for your child. We have a wide range of tutors and are confident that we will be able to find a match.
Please check the boxes at the bottom of the page if you give the AAC permission to use your child's likeness in photographs or videos. Please select neither if you do not give the AAC permission.
Terms of Service:
With acceptance to attend the AAC, I hereby release the Academic Achievement Center and all personnel, partners, and sponsors connected with the AAC or the programs it offers, from any liability, illness, or property damage that I sustain during my participation in this program or that is in any way related to this program. I understand that this applies to myself, my child, and our respective successors and personal representatives. I hereby authorize the directors and staff of the AAC to provide or retain medical care and to obtain emergency medical attention. My child is physically fit to participate in program activities, according to our family physician. By signing this form, I agree to the terms of service. I understand that I am responsible to provide any medication and maintain the supply as needed; to notify the AAC in writing of any changes in medication or prescriber; to pick up all unused medication by the last day of the program my child is enrolled in. I understand that my child will be held to behavior standards, and students who refuse to abide by the behavior guidelines may forfeit their privilege to attend the programs offered by the AAC and a phone call will be made for them to be picked up promptly by their parent or emergency contact. Dismissal from a program for not abiding by these guidelines also forfeits any request for reimbursement from the AAC.
